AT Freshman 25-26



All Freshmen, Transfers (undergraduate and graduate) and First-Year participants must complete the following:

Informed Consent and Education Form

  • This Form must be completed by every Student Athlete prior to the start of practices every year  

Sickle Cell

  • All Student Athletes must provide the results of their Sickle Cell Test

  • Must be completed by every Student Athlete one time prior to the start of practices 

  • Additional information can be found via the Sickle Cell Information

    • See "ATS" for additional information on how to upload results

Concussion

  • Concussion Baseline Sway Testing

    • The Sports Medicine Staff will be coordinating testing times with your coach. You will be receiving an email from your coach about your team's testing times

Banned Substances 

  • The NCAA has a yearly banned substance list, please check if any medications or dietary supplements you are taking fall into this list
    • If you taking medication for Attention Deficit Hyperactivity Disorder (ADHD) or Spironolactone, click here
    • If you are prescribed any of the other medications listed on the banned substance list, you will need pre-approval from the NCAA. Please notify the Athletic Training Staff and have your prescriber fill out this form
    • If you are taking dietary supplements (i.e. pre workout. mass gainer etc), email the Athletic Training Staff at sportsmedicine@marywood.edu the supplement so that it can be checked

ATS

  • ATS is the Electronic Medical Records software used by the Athletic Training Staff. All Student Athletes must create an account (preferably using the m.marywood.edu email address, if available). How to create an account for a Freshman, Transfer or First-Year Participant.

  • Freshmen, Transfers and First Time participants must submit both pages of the Marywood Athletics Physical found on ATS

  • Vaccination Status

    • If you receive a Covid-19 vaccination during the school year, please update the Screen/Tests tab by clicking "Add new Vaccination" and then email the Athletic Training Staff at sportsmedicine@marywood.edu to make them aware
