This form is not an official university application. We always suggest that in addition to completing a prospective athlete form/questionnaire below that you email the head coach to introduce yourself as well. Those prospects with general inquiries may email athletics@marywood.edu. 

NOTE: With the exception of Cross Country and Track and Field, if you are a dual- or multi-sport athlete (e.g. Men's Soccer and Baseball), you will need to complete one form for each sport you are interested in.
